How It Works

Getting a physical here takes a little planning on your end and about half an hour in our office.

01

Check your form

Read through the paperwork before you book. Fill in the parts meant for you or a parent, and note anything it asks for beyond the exam itself, so a single visit covers everything your organization needs.

02

Book a time

Choose a weekday or Saturday time online that fits around work, practice or classes. If a deadline is close, phone us so we can look for the soonest opening that suits your schedule.

03

Leave with it done

Your provider reviews your history, completes the hands-on exam and fills in the provider sections of your form in the same visit. You head out with the paperwork finished and ready to hand in.

Before, During and After Your Visit

Before Your Visit
  • Fill in the sections of your form meant for you or a parent, then bring the form itself along with a photo ID.
  • Jot down current medications, allergies and any past injuries, surgeries or conditions your provider should know about.
  • Wear comfortable clothes you can move in. Free private parking is on site, so arriving a few minutes early is easy.
During Your Visit
  • Your provider reviews your health history and checks vitals such as blood pressure, pulse, height and weight.
  • The hands-on exam follows the items your form lists, which for a sports form often include heart, lungs, joints and posture.
  • Ask questions as you go. The appointment is 30 minutes, so there is time to talk through anything on your mind.
After Your Visit
  • Look over every page your organization needs and confirm each one is signed and filled in before you head out.
  • Turn in the original to your coach, school, camp or employer, and keep a photo or copy for your own records.
  • If your provider suggested a follow-up, text or call us to plan that next step together.

When Will I See Results?

During the exam

Your provider talks through what they find as they go, so nothing on the form comes as a surprise.

Before you go

Your completed form is in hand, signed and ready to turn in.

If something needs a closer look

You hear it plainly before you head out, along with a clear next step you can act on.

Next season or new job

Check the date rules on your form and book again when the next one is due.

Frequently Asked Questions

How much does a physical cost without insurance?
A work, sports or camp physical at The Confidence Lab is $40. We are a self-pay practice and do not accept insurance, so that is the price for your exam and your completed form. We accept HSA and FSA cards for eligible services, so check with your plan about using yours for a required physical.
How soon can I get an appointment?
Open times show as soon as you start booking online, including weekday and Saturday hours, so you can grab the earliest slot that fits. If a school, camp or job deadline is close, call us for help finding the soonest time we have. Because the form is finished at the visit, you are not waiting on paperwork afterward.
Will my form be completed the same day?
Yes. Completing your form during the visit is the point of a physical here. Your provider fills in the exam sections and signs where the form asks for a provider signature before you leave, so there is no second trip and nothing to wait on by mail or fax. Bring the original if your organization wants it back.
How long does a physical take?
Plan on 30 minutes with your provider. Filling in your part of the form ahead of time and bringing a list of medications keeps things moving, so most of that time goes to the exam itself and to any questions you have. Booking for more than one family member? Schedule the appointments back to back.
What happens during a physical exam?
A physical is a focused medical exam that confirms you are ready for a job, a sport or a camp program, recorded on that organization's form. Your provider goes over your health history, takes your blood pressure and heart rate, examines what the form lists and notes the findings. For most people it feels like a relaxed conversation plus a quick hands-on check.
Is the price the same for work, sports and camp physicals?
Yes. Every physical here is $40, whether the form comes from an employer, a school team or a camp. It is the same exam with the same attention from your provider; only the form and the items it lists change, so you can book without guessing which version you need.
Can I use my HSA or FSA card for a physical?
HSA and FSA cards are accepted for eligible services. At $40, a physical is a small, predictable expense either way. If you are unsure whether your plan counts a required physical as eligible, your plan administrator can confirm it.
What does the exam price include?
The $40 price covers your visit with a provider: a review of your health history, vital signs, the hands-on exam your form calls for and completion of the form itself. If your organization asks for anything outside a standard physical, phone us before you book to walk through it together.
Do you do school sports physicals?
Yes. We complete sports physicals for school teams, club leagues and recreational programs. Bring the exact form your school or league uses, since many have their own version, and your provider completes the exam sections and signs it before you leave. That way your athlete can hand it in at the next practice.
Do you do pre-employment physicals?
Yes. If a new job or your current employer asks for a physical, bring the employer's form or written requirements to your visit. Your provider completes the exam and documents what the employer requires. If the paperwork lists something beyond a standard exam, send it our way ahead of time and we will look it over together.
Can my child get a camp physical here?
Yes. Day and overnight camps often send their own health form. Bring that form, any immunization record the camp requests and a list of medications, and your provider completes the exam portion during the visit. Booking a few weeks ahead of the camp deadline leaves room if the form turns out to need anything extra.
Does a physical hurt?
A standard physical is non-invasive. Your provider listens to your heart and lungs, checks blood pressure with a cuff, looks at posture and joints and asks about your history. You may feel light pressure at points during the exam, and nothing involves needles unless your form asks for something extra, which we talk through with you first.
What if my provider finds something during the exam?
Your provider explains anything worth a closer look before your visit ends and what it means for your form. Many findings simply get noted on the paperwork. If something calls for follow-up, you hear the next step plainly and we help you plan it, so you walk out knowing exactly what comes next.
How often do I need a new physical?
It depends on who is asking. Many schools and leagues want a new sports physical each year or season, camps often ask for one completed within the past 12 months and employers set their own rules. Check the date requirements printed on your form, and book your next visit once the current one is close to expiring.
Who will perform my physical?
Physicals are performed by board-certified or licensed providers, and the practice is owned by nurse practitioners who do the clinical work themselves. The provider who examines you is the one who completes and signs your form, so any question about your findings goes straight to the person who saw you.
Can I book online?
Yes. Booking online asks for a time, your details and a card on file to hold the appointment. If you would rather talk to someone, call or text (614) 219-9983 during business hours and we will find a time with you. Either way, bring your form to the visit.
Where are you located, and is parking easy?
You will find us at 4038 Powell Rd, Powell, OH 43065, an easy drive from Dublin, Lewis Center, Delaware, Worthington and Columbus. Free private parking is available for every client, so getting in and out around school pickup or a lunch break is simple.
Can I add a vitamin injection to my visit?
Yes. You can book a vitamin injection or an IV hydration appointment alongside your physical if you want extra support heading into a season or a new job. Book each one online back to back, and mention it when you check in so both visits run smoothly.
